Follow-Up Care After Inpatient Rehabilitation

Leaving the hospital or inpatient rehabilitation facility is an important milestone, but recovery doesn’t stop at discharge. At Rehab Medicine of the Carolinas, we help patients continue their rehabilitation journey through outpatient care in a private, multidisciplinary clinic. Our team builds on the team-based approach established during inpatient rehabilitation, providing continued medical support as patients work toward their recovery goals.

The transition home can feel overwhelming, especially after a long or complicated hospital stay. At Rehab Medicine of the Carolinas, we provide outpatient medical rehabilitation care for patients who continue to have complex needs after discharge.

We are proud to offer on-site physical therapy in a fully equipped, private rehabilitation gym where patients can feel comfortable and supported throughout their recovery. Our physical therapists work closely with each patient to develop individualized treatment plans that reflect their unique needs and goals, helping them work toward improved strength, mobility, and function.

Once patients meet their rehabilitation milestones through outpatient therapy, care doesn’t stop there. Treatment continues under direct physician supervision, ensuring continuity of care and a seamless transition as patients progress toward greater independence and long-term wellness. Our interdisciplinary approach allows for consistent monitoring and adjustments as needed to optimize outcomes.

Our medical team brings decades of combined experience in acute inpatient rehabilitation. We understand the unique physical, emotional, and medical challenges patients may face after hospital discharge. We are especially attentive to patients transitioning from intensive care or recovering from surgery, providing a compassionate and clinically sound environment for continued rehabilitation.
