Pediatric to Adult Transitions Program
The Transitions Program at Rehab Medicine of the Carolinas provides streamlined care from pediatrics to adulthood for patients with pediatric disorders.
Some of the conditions treated may include: congenital extremity deficiencies, Down’s Syndrome, Cerebral Palsy, Spina Bifida, scoliosis, traumatic conditions, burns, muscular dystrophies, and developmental delay.
The Individuals with Disabilities Education Act is a federal law that requires intervention for children with developmental delay in the first few years of life. Care for pediatric patients is oftentimes streamlined and easy to obtain; however, as pediatric patients enter legal “adulthood” the care must be transitioned to a physician that is trained to manage the unique needs of these special populations.
